Step One
Attend one of our free Informational Seminars.

Step Two
Complete the Health Questionnaire

Click on the link above to download the Health Questionnaire and bring with you to your first appointment.

 Step Three
If you plan to go through your health insurance for coverage of surgery, we will verify your benefits for surgery and then communicate to you both your benefits and the specific criteria that must be met for your insurance plan to cover surgery. Please see the financing/insurance section of our site for detailed information.

If you plan to self pay, please contact the surgeon’s office directly to set up your consultation.

 Step Four (insurance patients only)
Once all of your insurance company’s criteria have been met, a letter will be sent to your insurance carrier for pre-determination. We encourage you to contact us at any point throughout the process at 615-326-2502 should you have questions.

Step Five
All patients will have a surgeon consultation, a nutritional evaluation, a psychological evaluation and attend a pre-op class prior to surgery. We will advise you when to schedule these appointments.

Step Six (this step may be completed at any stage of the process)
All patients are expected to attend a minimum of one support group prior to surgery. Please click here for the support group information.
